Client will explore and process clinical thoughts, triggers, and situations that contribute to
his frustration and depressed thoughts. Client will identify troublesome feelings and link
feelings to specific triggers and areas of vulnerability as evidenced by self-report,
observations and collateral professionals processing his emotional pain for past and present
that contribute to feelings of sadness. Also identify positive events.
Therapeutic interventions provided:
TBS worker conducted group with client at community group facility. TBS worker educated
the client on strategies to improve focus and manage inattention, including creating
structured routines and using organizational tools such as checklists and visual schedules.
Techniques such as breaking tasks into smaller, manageable steps and incorporating
regular breaks were introduced. Additionally, the client was taught mindfulness exercises to
help stay present and improve sustained attention during activities.
Response to interventions/progress towards goals & objectives, including client’s
assessment of progress, if applicable: The client was engaged and actively participated

in the session. They reported that the cognitive restructuring techniques helped them
recognize and challenge negative thoughts, providing some relief from feelings of sadness
and hopelessness. The client also found the relaxation techniques helpful in managing their
anxiety. They expressed a desire to continue applying the strategies in their daily life and
reported feeling a slight improvement in their mood. Progress was made towards increasing
emotional resilience and managing depressive symptoms, with the client demonstrating a
willingness to practice the techniques discussed in the session.
