1. Description of the Incident

On the morning of July 12, 2025, during the second recess period, an accident occurred involving a
student, Emma R., aged 9, in Grade 4. The student was playing on the monkey bars when she lost her
grip and fell to the ground. The fall was from approximately 1.5 meters.

2. Immediate Response

Playground supervisor Ms. Clara Jennings witnessed the fall and immediately approached the student.
Emma was conscious but visibly in pain and clutching her right arm. A first aid response was initiated,
and the school nurse, Mr. Daniel Cho, was called to the scene within two minutes. The nurse
immobilized the arm and advised that Emma be taken for further medical evaluation.

Emma’s parents were contacted at 11:25 AM and arrived at the school at 11:45 AM. They took her to
the local urgent care clinic for assessment.

good condition, securely anchored, and compliant with safety standards. The ground surface beneath
the bars is padded with rubber mulch, which helped minimize the impact of the fall.

 Staff will increase supervision near climbing equipment during recess.
