AFFIDAVIT
I, ________________ of legal age, Filipino and a resident of Purok 27-A,
Malagamot, Panacan, Cebu City after having been sworn to in accordance with law,
depose and say;
1. That I am the driver of an Isuzu Platform Truck with Plate Number LVD-106
owned by _____________;
2. That I am a holder of a Professional Drivers License with No. L02-06003700 which will expire on November 24, 2014;

3. That I figured in an accident in which the circumstances are as follows:
On February 10, 2014 at around 10:30 in the morning,I was driving
along Mamay Road coming from JP Laurel Avenue and heading towards CP
Garcia Highway, with the vehicle being driven by ______________ which is a
Honda Civic Sedan with Plate Number ZTM-489 (PVT) owned by
__________________, ahead of me.
Upon reaching the injunction to Victoria Heights Subd. Road,
____________ executed a right turn and while on the process,the left side rear
corner portion of the Honda Civic Sedan that he was driving was bumped
from behind by the right side front corner bumper portion of the vehicle that I
was driving.
4. As a result, the Honda Civic Sedan being driven by _____________ incurred
damages;
5. That I am executing this affidavit to attest to the truth of the foregoing
facts surrounding the accident and such other purpose that the same may
serve.
