PETITION
PETITIONERS, through counsel, and to this Honorable
Court, most respectfully state:
1.
Petitioner KRISTINE ABEGAIL GODALLE LANUZA , of
legal age, Filipino Citizen, and resident of Unit 514 Ventura
Residences, 646 MV Delos Santos St., Manila.
2.
Respondent LOCAL CIVIL REGISTRAR OF Puerto
Princesa City, Palawan, is the head of the government office
charged with the registration, recording, and issuance of
documents and other papers relative to the status of a
person in the City of Manila, National Capital Region, and
could be served with summons and other court processes at
its official address at the Office of the Local Civil Registrar of
Puerto Princesa City, Palawan.
FACTUAL ANTECEDENTS

Petitioner is of the age which she may adopt as
provided by the law. The authenticated birth certificate of
the petitioner hereto attached as Annex A.
4. Petitioner is in possession of full civil capacity and legal
rights; of good moral character; have not been convicted of
any crime involving moral turpitude as evidenced by NBI
Clearance and Police Clearance hereto attached as Annexes
B and B 1, and Affidavits of Character Reference
marked as Annexes C, D and E; are emotionally and
psychologically capable of caring for the adoptee; not
otherwise disqualified by law to adopt as evidenced by the
Medical Certificate hereto attached as Annexes F; and in a
position to support and care for the adoptee in keeping with
the means of the family as shown in the Latest Income Tax
Return marked as Annex G and other proof of income
hereto attached as Annexes H to H-2.

Petitioner attended the adoption forum conducted at
the Kaisahang Buhay Foundation last January 24, 2016. A
certification of attendance is attached hereto as Annex I.
6. The adoptee was left under the custody of the Department of
Social Welfare and Development after a rescue operation
conducted by the Philippine National Police for street
children last October 30, 2014 at Recto Avenue, Manila. The
authorities failed to locate the parents or guardian of the
adoptee even through the thorough effort of the former.
Attached hereto are the Police Report and proof of turnover
of the custody of the adoptee as Annex J and J-1.
7. Petitioner likewise was declared qualified to adopt by the
Department of Social Welfare and Development through the
Consent to Adoption issued and signed by Undersecretary of
Operations and Program Groups Parisha Hashim Taradji last
June 30, 2016. Attached hereto as Annex K.
8. Petitioners guarantee that Adoptee will have a secure and
bright future considering that both Petitioners have a regular
and steady source of income.
9. The adoption will serve the best interest and well-being of
the Adoptee, who has not been adopted before nor has been
disqualified by law to be adopted and who does not possess
any property.
PRAYER
WHEREFORE, premises considered, petitioners pray
that after due notice, publication, and hearing, judgment be
rendered:
a. Henceforth, the adoptee Nia Berdonar be for all
legal intents and purposes, be the child of the
Petitioner;
b. Establishing the status of Nia Berdonar as the
legitimate child of Petitioner; and
c. Ordering the Local Civil Registrar of Puerto
Princesa City, Palawan to change adoptees name
in her Certificate of Live Birth from Nia Berdonar
to NIA LANUZA.
Other relief just and equitable are likewise prayed for.
