AFFIDAVIT OF INSERTION

I, VERONICA D. DARTE , Filipino citizen, single, of legal age and


resident of B28 L1 Ph 7 Golden City, Dasmarias City, after being duly sworn
to in accordance with law, depose and state: That - -

1. That I am the BUYER/VENDEE in the Deed of Absolute


Sale of Real property executed between me and Diomedes R. Bragado
with Transfer of Certificate of Title: 057-2012019750, notarized by
Atty. Victor B. Borromeo as per Doc. No. 2, Page No. 1, Book No.
1, Series of 2016.

2. Unfortunately, my civil status as single and legal status of


legal age was not declared in the above said document.

3. Through this affidavit, I hereby declare under oath that my legal


status is of legal age and my civil status is single and request
such to be inserted or declared in the above said document.

4. I am executing this affidavit to attest to the truth and


veracity of the foregoing and for whatever legal purpose the same may
serve;

IN WITNESS WHEREOF, I have hereunto affixed my signature this 30th


day of September 2016 in Dasmarias City, Cavite.
