AFFIDAVIT OF SEPARATION FROM EMPLOYMENT

I, Ruby F. Orillan , of legal age, Filipino, and with office address at Gen.
Luna St., Estaka, Dipolog City after having been duly sworn in accordance with
law depose and say that:

1. I am the BRANCH MANAGER of Davids Salon, with office address at


Gen. Luna St., Estaka, Dipolog City and the authorized representative of
said company engaged in the business of hairstyling and beauty application
of world-class business;

2. As a branch manager I know a fact that Johnly A. Mapili was our Junior
Hairstylist since May 30, 2013 and ended last November 18, 2015 due to
Absence Without Leave (AWOL) for unknown reason, and never showed
up to the present time;

3. That I am executing this Affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose it may serve.
