MATERIEL WING Savings and Loan Association Inc. with Insurance Policy No.

2. This BMP Certificate was lost during the onslaught of Super Typhoon Yolanda on
November 8, 2013;

3. Diligently, we looked for it but we consider it now legally lost beyond ways;

4. I have executed this instrument to attest to the truth and veracity of the foregoing
facts for my Hospital Claim and whatever purpose the same shall serve.
