encouraged her to find her strength in confidence as well

Clinical Instructor Comments (Please include strengths as well as needs for improvement. There must be comments to
elaborate on any objective marked “unsatisfactory”.

Megan comes prepared for each clinical day and is dressed in a professional manner (ie. clean uniform, name badge presented at eye
level and hair tied back). Megan builds and maintains therapeutic relationships by asking each resident what name they prefer to be
addressed, spending time learning about the residents backgrounds (history and personal), being respectful of their homes, and
developing partnerships with the residents when providing care. Megan does really well at introducing herself and her designation
each and every week at clinical as the residents may be living with dementia and/or have many student visitors. Megan ensures she
reviews the individualized plan of care for each resident she assists and promotes the residents' independence by enabling each
person to do as much as they can on their own before providing assistance. Megan needs to ensure that prior to each time she
provides assistance she communicates with the resident and asks permission when appropriate. Megan con further promote safety in
the clinical environment by raising/lowering beds, ensuring privacy by closing doors, continue to follow the care plans thoroughly,
and apply proper ordered restraints, etc. Megan does good at recognizing her own limitations and seeks help when necessary. Megan
should look for new opportunities to practice her skills and maximize her competence. Keep up the great work thus far.
