AFFIDAVIT OF WITNESS

I, Christmer C. Caňete, of legal age, single, Filipino and a resident of


Buru-un, Purok 7, Iligan City, after being sworn to in accordance with law,
depose and state:

1. I am the son of Vicente M. Caňete Jr., complainant of this case;


2. That at the time of the incident, I was a passenger of our vehicle;
3. That my father before and during the incident was driving responsibly
with due regard for all circumstance and did not violate any traffic rules
or regulations;
4. That at the middle portion of the intersection of Manuel L. Quezon
Avenue and Mariano Badelles Street, located in Barangay Poblacion,
Iligan City, we were suddenly hit on the right part of our vehicle
identified by the Traffic Police of Iligan City as Judy S. Becoy of Rural
Transit Mindanao, Inc. of Bulua Cagayan de Oro City;
5. That the hitting was sudden and surprised us;
6. That after the incident, we were rushed to the Adventist Medical
Hospital so that the injuries we have sustained will be attended;
7. That the injuries I have sustained required 5 days of Hospitalization
and self medication;
8. That I am executing this Affidavit to attest to the truthfulness of the
following statements, to serve as an evidence for the prosecution of the
driver of the Rural Transit Mindanao, Inc., and for whatever legal purposes may
serve me best.
