AFFIDAVIT

THAT I, SYLBETH ANGUIT TALIBONG, Filipino citizen, of legal age, married


but separated de facto and presently residing at Barangay Bago, Sindangan, Zamboanga
del Norte, Philippines, after being duly sworn in accordance with law, depose and state
that:

1. On August 17, 2003, I was married to ROLANDO TALIBONG JR. in a church


wedding ceremony in Siare, Sindangan, Zamboanga del Norte;

2. During our marriage, we were blessed with two (2) children, namely:
SHYLKENT ANGUIT TALIBONG (15) years old and KAYE ANGUIT TALIBONG
(6) years old, they were born on January 27, 2003 and August 31, 2011, respectively;

3. I am now separated with my husband for almost seven (7) years since 2011, he
abandoned us without providing any financial support and I was left solo or alone with the
responsibility of taking good care and providing support for our children;

4. That I am executing this sworn affidavit to attest the truth of all the foregoing
facts and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 13th day of February,
2018 in Sindangan, Zamboanga del Norte, Philippines.
