PRELIMINARIES: Affiant was informed on his obligation to tell the truth and nothing but
the truth. The questions were translated into a dialect clearly understood by the affiant.

A: Yes I do.

2. Q: Do you swear to truthfully answer the questions asked of you, fully conscious
that you do so under oath and that you may face criminal liability for false
testimony or perjury?

A: I am RONA B. OCLARIT, 38 years old, married to RAMEL G. OCLARIT, and a
resident of Purok 3, Humilog, RTR, Agusan del Norte. I am currently unemployed
while my husband is a baker.

4. Q: Under what circumstance did you acquire the facts upon which you testify?
A: I am testifying in my capacity as the complainant in this case.

5. Q: What Complaint are you contemplating to file against the respondent?


A: I am filing a criminal complaint of Violation to Section 358 - Slander or Oral
Defamation of the Act No. 3815 otherwise known as the REVISED PENAL
CODE OF THE PHILIPPINES against ELSA ZERINA, also a resident of Purok 3,
Humilog, RTR, Agusan del Norte.

A: The offense took place within our neighborhood, specifically in the locality of
