DISCREPANCY

I, MARY RENALIZA MARZ AVELIN, of legal age, single,


Filipino and a resident of Lot 28, Zone 4,Block 41, Welfareville
Compound, Brgy Addition Hills, Mandaluyong City, having been
duly sworn in accordance with law, hereby depose and state
THAT:

1. I was born on March 31,1995 at our residence Lot


28,Zone 4,Block 41, Welfareville Compound, Brgy
Addition Hills, Mandaluyong City;

2. My first name “MARY RENALIZA” had been erroneously


misspelled in my Certificate of Live Birth as “MARY
RINALIZA”.

3. My true name which I have been using since my


childhood in my school records and other documents is
“MARY RENALIZA MARZ AVELIN” and not “MARY
RINALIZA MARZ AVELIN” as appears in my NSO
Certificate of Live Birth.

4. I execute this Affidavit of Discrepancy to attest to the


truth of the foregoing circumstances and to explain the
discrepancy in my NSO Certificate of Live Birth with
regard to my first name.

IN WITNESS WHEREOF, I have hereunto set my hand
