That I am the same person who executed the foregoing affidavit;
That I secure a copy of my Certificate of Live Birth from the Philippine
Statistics Authority of Pagadian City;
That in the said Certificate of Live Birth, my middle name and family
name erroneously appears as “ADELPHA BENDEJO EBAL” instead of
“ADELPHA BEDRIJO EBOL”, which is my correct and complete family name
as appearing in all my other pertinent documents;
That the said discrepancy was merely a typographical error and not
intentionally done to neither commit wrong nor perpetuate fraud or other evil
motive;
That the middle name and family name “ADELPHA BENDEJO EBAL”, as
appearing in the Certificate of Live Birth, and the name “ADELPHA BEDRIJO
EBOL” which is my correct and complete name, refers to one and the same
person;
That I hereby declare, affirm and confirm that my true and correct name
is “ADELPHA BEDRIJO EBOL”;
