AFFIDAVIT OF LOSS

I, SHERLITO M. COLUMNAS JR, of legal age, married, Filipino citizen, and a


resident of Cabacungan, Cabilao, Loon, Bohol, Philippines, after being duly sworn to
in accordance with law, depose and state that:

(1) I am a passer of the Real Estate Broker Examination conducted by the


Professional Regulatory Commissionon on March 17, 2013;

(2) Being a licensed Real Estate Broker, I was issued a PRC ID with no. 0015865
by the PRC;

(3) I lost the said PRC ID which is needed for the renewal of my PRC licensed;

(4) Despite diligent search and effort to locate the said PRC ID, the same could
not be found such that I now consider it lost beyond recovery;

(7) As such, I am executing this Affidavit of Loss to attest to the foregoing and for
any legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 10th day of July 2018
in Tagbilaran City.
