In December 2017, AAA's wife was involved in a minor accident while driving the vehicle in Abuyog, Leyte, causing dents and scratches on the front left side. 3. Prior to the accident, the vehicle was painted blue, but AAA had it repaired and repainted metallic red by vvv Welding Shop in March 2018, despite the color on the registration still stating blue.

AFFIDAVIT

I AAA, Filipino, of legal age, and a resident of, McArthur, Leyte after having been
duly sworn to in accordance with law do hereby depose and say THAT:

1. I am the registered owner of a motor vehicle by virtue of a Deed of Sale executed


by BBB in favor of me, which is particularly described as follows:

Make: Hyundai Series: EON 0.8L GL MT


Engine No: xxxxxx Chassis No: xxxxx

2. Last December 2017, while my wife CCCC was driving the above-described motor
vehicle in Abuyog, Leyte, she met a slight vehicular accident that caused dents
and scratches on the front left side of the above-described vehicle;

3. Prior and during the said incident the above-described motor vehicles was painted
color blue;

4. I decided to secure the services of a professional to repair the said motor vehicle
sometime last February 2018, and included the repainting of the said motor vehicle
from blue to metallic red;

5. For this repair and repainting we secured the services of vvv Welding Shop owned
by vvv with business address at Tolosa, Leyte;

6. The said repair and repainting has been promptly finished last March 2018 and
now the above-described motor vehicle’s color is METALLIC RED despite the fact
that in the O.R. of the said motor vehicle the color indicated therein is Blue;

7. That I am executing this affidavit freely and voluntarily in order to establish the truth
of the foregoing facts and for whatever legal purpose this may serve.
