Permit to work system
Permit-to-work System is a formal written system used to control
certain types of work which are potentially hazardous. It is also a
means of communication among site personnel to ensure all necessary
safety precautions are taken before commencing such work.

Procedure:

General Work Permit initiating
Procedure:
The Issuer shall visit site and ensure isolation of all Hazardous
energy sources are completed by respective authorized
isolators. He shall give clearance to start the work and sign the
permit.
Duly signed & approved work permit along with associated
permits, TBT shall be kept at site where work group is
performing the job.
Initiator is responsible to maintain the status quo for control
measures stated in the work permit during execution of work
safely.
Co-permittee is equally responsible to maintain the status quo
for control measures stated in the / work permit during
execution of work safely.

Procedure:
Closure of General Work
Permits
• After Job completion, the Initiator ensure
that site is clear and ensure that
isolations made, can be restored.
• After closing hand over the Permit copy
(First copy) to Issuer to close the permit.
• The Issuer shall direct authorized
isolators to carry out restoration of
isolation
• Respective authorized isolators shall
carry out restoration.
• Issuer shall ensure restoration of all
isolation mentioned are completed and
shall sign the permit.
• He shall declare equipment ready to start
and shall inform the custodian.

Procedure:
Distribution of permit
1. First copy (white) of the work permit and
any attached permit(s) shall be with initiator
/ co-permittee and shall be at job site.
2. The second copy (yellow) shall be with
issuer and shall be displayed at issuer’s
designated area. (Office/control room).
3. The third copy (pink) will remain in the
permit book and shall be available with
custodian for office record.
4. On completion of the work, all the three
copies shall be signed by the concerned
persons.
5. The completed copy of the permit shall be
retained at respective facilities for office
record purpose along with all associated
permits/documents for 1 year
6. Closing time in permit should be within 30
minutes after work completion.

Renewal of General
Work Permit
1. Permit shall remain valid for
one shift, including General
Shift.
2. New work permits shall be
issued whenever Initiator or
Co-Permittee is changed for
any reason (even before
completion of the shift),
3. A permit is valid for a specific
scope of works, on specific
locations with same team
within a specified period.

4. It is valid till the end of the
shift in which it is issued.
5. If the work extends beyond
the shift it is issued, permit
must be revalidated or
renewed.
Permit to work system
Procedure for renewal of General work
permit:
Issuer shall inform the custodian. He will make a site visit if the work
permit is to be renewed.
Initiator shall brief the work group regarding the hazards and the risk
control plan/ safeguards and obtain signature of Co-permittee.
Issuer, Initiator and Co-Permittee shall sign all the three copies of permit
and shall handover the 1st copy to initiator
In the event of change in work group members, the Initiator shall inform
the Issuer about the change.
He shall also explain the hazards and safety measures to new person(s) in
the work group. Undertaking of new members shall be obtained on TBT
Initiator shall restart work after renewal by issuer.
IMPORTANT: ANY CHANGES IN THE WORK
PERMIT MUST REFLECT ON ALL THE
THREE COPIES

Deviation Process:
Deviations from this standard must be reviewed by the Safety
Leader and authorized by the Cluster head in consultation with
operation Head.
Deviations must be documented, and documentation must
include the relevant facts supporting the deviation decision.
Deviation authorization must be renewed periodically as
necessary as and not later than one year.

Emergency Deviation:
Emergency deviations must be authorized by the Custodian
Emergency deviations shall be authorized only where it is not
feasible to comply with a requirement in this standard.
Emergency deviations shall be short in duration, not to exceed
the time to perform the task in hand.
Custodian will keep ESTATE MANAGER and Group Safety Leader
informed verbally during such times.
As a follow-up, the deviation must be documented in prescribed
format and a copy of the deviation must be sent to the Estate
Manager in consultation with Brookfield Operations Head.
The documentation must include the relevant facts supporting
the deviation decision and the interim measures to be put in
place to achieve acceptable levels of control measures.

Inspection:
Ensure Portable Power tools, Distribution board used by initiator for his
fitness and inspect it with IMS checklists.
Ensure AWP, MEWP working platforms are Third party inspection done and
valid till date.
Check portable tools are in good condition before use, with IMS checklists.
Check Whether right tool used for right job.
Check Eye bolt, safety harness and lifting tools are Third party certified.

Safe work instruction:
Hot Work
The surrounding area should be checked & flammable materials
removed.
The welding spark should be controlled using wet welding curtain
provided.
The proper lighting & proper means of exit should be available /
provided.
The welding cables should be in good condition.
The electrical power tools should have proper earthing & three pin plug
tops.
Ensure; Supply should relate to ELCB. The fire watch should be
provided & trained in use of fire extinguisher.
The gas cylinder should be mounted on trolley & secured.
Provision of adequate and appropriate fire extinguisher should be
ensured.
The operators should be trained on the process as well as on the high
risks involved. 16

Safe work instruction:
Working at Height
The work area should be barricaded, caution signage displayed.
Durable ladder/scaffold should be provided for work. Ensure all the
checkpoints.
Ladder should be checked using checklist for its fitness.
A standby person should be deputed to watch & warn any person who
comes near the place where the activity is performed.
Ensure; Operator is wearing all the necessary PPE.

Safe work instruction:
Confined Space
Confined Space refers to an area which is enclosed with limited access
which makes it dangerous.
The confined space should be left open for fresh air circulation before
entry. The surrounding area should be checked & barricaded
The drain, pit, sewer & floor opening/ confined space should be
checked for presence of toxic & flammable vapors
Oxygen cylinder should be carried while entering the confined space if
required Proper ventilation & lighting should be made available
standby person should be trained & deputed outside to watch the
activity Proper scaffold / ladder should be provided & tied at the top if
required
Safety harness should be used & lifeline anchored above the work
level if required
