Gilenya is a brand-name prescription drug prescribed to patients as young as 10 with recurring multiple sclerosis (MS), a disease in which the immune system attacks nerve cells in the brain, spinal cord, and optic nerve. The swelling caused by the immune reaction destroys the outer protective layer of nerve cells, called the myelin sheath. As the myelin degenerates, nerves no longer send signals correctly and eventually die. Scar tissue (sclerosis) replaces the dead nerve tissue.
While the exact mechanism is not well understood, fingolimod, the active ingredient in Gilenya, is believed to prevent MS attacks by blocking the ability of white blood cells to cross over into the central nervous system.
Gilenya is taken as a capsule daily. Because Gilenya slows down the heart, the first dose is always carefully monitored. An electrocardiogram (EEG) is taken immediately before the first dose and at the end of the observation period.
Gilenya suppresses the immune system, so Gilenya lowers the body's ability to prevent or fight infections while on the drug and for two months following its discontinuation. Patients on Gilenya are at a higher risk for infection and cancers.
