Store minter state in database

• EZID minter state is preserved as JSON entries within the MySQL database
• Minting action protected by database transactions to prevent race conditions, removing need for locks around createIdentifier
• Load database from existing minter state stored in Berkeley db
• Dump minter state to JSON
