AFFIDAVIT OF DESISTANCE

oath, depose and state, that –

1. I am one of the private complainants in Criminal Case No.

4. I am executing this Affidavit of Desistance to attest to


the truth of foregoing facts and hereby waive/quitclaim any and
all of my claims pertaining to the incident subject of the above-
mentioned case now and thereafter, be it
civil/criminal/administrative in nature.

5. The foregoing statement have fully been read by me and/


or read/translated to me both in Ilonggo dialect and English
which I understand and that the same is true and correct to the
best of my knowledge.
