AFFIDAVIT OF UNDERTAKING

I, _________________________________, of legal age, single/married, Filipino,


with office/residence and postal address at ___________________________________, in
accordance with law, do hereby depose and say – THAT:

1. I am the owner/manager/duly authorized representative of business establishment


named ___________________________________________________ located at
___________________________________________;

2. I am securing a business permit and license from the Business Permits and Licensing
Office (BPLO), City Government of Muntinlupa for the aforesaid business;

3. I am aware of the terms and conditions laid down by laws for the promotion of
health and safety, improvements of public morals, maintenance of peace and order,
preservation of comfort and convenience of the inhabitants and the promotion of
general welfare in general;

4. Thus, in adherence to the foregoing undertake that the business SHALL:

a. Not engage nor participate in the promotion of prostitution;


b. Strictly limit the function of female waitresses to serving
beer/food/orders of clients;
c. Strictly prohibit female employees from sitting/staying at tables
with customers/client for any reason or purpose whatsoever;
d. Not have “VIP” rooms;
e. Ensure that within fifteen (15) days after its opening, secure
occupational or working permit of all masseurs/ entertainers and
employees from the City Government;
f. Secure sanitary permit within fifteen (15) days from its operation;
g. Submit health permit duly issued by the DOH of all masseurs/
entertainers within thirty (30) days from its operation; and
h. Submit Muntinlupa Restaurant and Bar Association Certificate
(MRBA); and
i. Submit to and comply with the rules and regulations that may be
issued by the proper authorities; and

5. In case of violation of any of the foregoing, I interpose no objection to the automatic


suspension, cancellation or revocation, as the case may be, of my business permit
and license, and the subsequent closure of the above-cited business establishment, if
necessary;

6. That I execute this affidavit in order to attest the truth of the above facts for all legal
intents and purposes it may serve;
