PELIGRINO, in my certificate of eligibility to KRISTENE IVY ROSALES SIMON since
the name which I’ve used during my application, which I carried out from my school records
was not of a match with that of my birth certificate. And to reduce a much larger and further
problem I chose to follow what is written in my birth certificate.

Attached hereby are photocopies of my certificate of eligibility, birth certificate and my


corrected school records which maybe of your basis.
