SUBMISSION and UPDATING OF 201 FILES
1. Download PDS Form or CS Form 2012, revised 2017 – Two copies ( 1 Handwritten and 1
     computer written). Original Picture in passport size GOA uniform in white background.
       Signature must be in black    ball pen.
2. Insert to the table of contents the following in one (1) original copy or authenticated or
        certified true copy;
       a. Attested appointment --- Original (initial appointment) to Present Appointment.
       b. Latest Mandatory/Relevant Trainings/Seminars --- CY 2017 to Present.
       c. Family Documents --- PSA Marriage Contract and PSA Birth Certificate of
           Dependents. (2017-Present) – documents already updated and submitted 2016
           below need not to comply.
       d. Commendations – In Present Rank.
       e. Updated Service Record – Original.
       f. Updated Itemized Leave Record with ISO Code – Original.
Note: Green – Certified or authenticated by any BFP officer.
      Blue -- Original Copies or Certified Copies or Authenticated by PSA office ONLY.
3. Submit the following in PDF Format (must be in Colored) – One (1) Folder Only in SOFT
       COPIES;     (scanning through cellphone application is not allowed).
       a. Attested appointment --- Original (initial appointment) to Present Appointment.
       b. Latest Mandatory/Relevant Trainings/Seminars --- CY 2017 to Present.
       c. Family Documents --- PSA Marriage Contract and PSA Birth Certificate of Dependents.
         (CY 2017 – Present)
       d. Commendations – In Present Rank.
       e. Original Updated Service Record.
       f. Original Updated Itemized Leave Record with ISO    Code.
Note: All documents must be inserted only to the table of content sent to your office and need
not to comply all the contents found in the table of contents for they are already existed in the
Regional Office Record Section. Example : Appointment documents must be inserted in the letter
H in the Table of contents.

5. Submit in Green EXPANDED folder and GREEN Tabbing letters (Complete from A to R).
