AFFIDAVIT FOR CORRECTION

I, ERNESTO MATIAS, of legal age, married, and a resident of Kalawag II, Isulan,
Sultan Kudarat, after having been duly sworn to in accordance with law depose and say:

1. That I am the registered owner of Lot No. 20, Pls-45 containing an area of FORTY
EIGHT THOUSAND SIX HUNDRED NINETY TWO (48, 692) SQUARE
METERS, more or less covered by Homestead Application No. V-4916;

2. That as per verification in the office of Department of Environment of Natural


Resources Lot No. 20, Pls-45 it was erroneously and advertently entered as Lot
No. 64, Pls-20-D-13;

3. That it was also proved in the Technical Description SIGNED BY Geodetic


Engineer Rogie D. Hagoriles that the correct description of my parcel of land is
Lot No. 64, Pls-208-D-13 instead of Lot No. 20, Pls-45;

4. That I am executing this affidavit to ask your good office to correct the lot
numbered entered in the Homestead Application No. V-4916 from Lot No. 20, Pls-
45 to Lot No. 64, Pls-208-D-13;

5. That also I am executing this affidavit to attest the truth of the above-foregoing
statements for whatever legal purposes it may serve best.

IN WITNESS HEREOF, I have hereunto set my hand this 29th day of February, 2016 in
Isulan, Sultan Kudarat.
