AFFIDAVIT OF TWO (2) DISINTERESTED PERSONS

WE, DANILO M. CORPUZ and ROMEO T. PIDENES, Filipinos, both legal


age, presently residing at Science City of Muñoz, Nueva Ecija, after having been sworn
to in accordance with law hereby depose and state, THAT:

1. We personally know the person of JOSE DOROTEO DE ASIS ROADILLA who


was born on September 09, 1961 to Spouses Demetrio Roadilla and Adelaida E.
de Asis at Macalelon, Quezon.

2. We personally knew his person because we are family friends of the Roadilla
family.

3. We knew that in his Marriage Certificate his name was erroneously written as
JOEY A. ROADILLA, in his Death Certificate his name was written as JOSE DE
ASIS ROADILLA, in his Certificate of Live Birth his name was written as JOSE
DOROTEO A. ROADILLA and in his Social Security System Record his name
was written as JOSE DOROTEO DE ASIS ROADILLA. .

4. Lately when he secure his birth certificate he discovered that his real name was
JOSE DOROTEO DE ASIS ROADILLA.

5. We knew that JOEY A. ROADILLA, JOSE DE ASIS ROADILLA, JOSE


DOROTEO A. ROADILLA, JOSE DOROTEO DE ASIS ROADILLA, the true
and correct one, refer to one and the same person.

6. We are not his relatives, either by affinity or consanguinity and we have executed
this joint affidavit without any promise of reward or money and will never be
benefitted by its execution.

7. We have executed this affidavit to attest to truthfulness of the foregoing facts and
for whatever legal purpose it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this August 5, 2019 at
Science City of Muñoz, Nueva Ecija.
