AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, MARIO C. VICENTE, 47 years old and GEORGE M. PEREZ, 36


years old, Filipinos and residents of Barangay Mapangpang, Lupao, Nueva Ecija
and after having been sworn to in accordance with law hereby depose and state,
THAT:

1. We personally know REYNALDO Y. BALADAD, resident of Barangay


Mapangpang, Lupao, Nueva Ecija.

2. We likewise know that he was raising 4 cows since 2012.

3. That on October 24, 2019, one of the cows died due to snakebite.

4. These facts, are of our own true and personal knowledge, for we are
barangay mates and long time friends of REYNALDO Y. BALADAD.

5. We executed this affidavit without promise of reward and that we will not
be benefited for its execution.

6. We have executed this affidavit to attest to the truthfulness of the foregoing


and to inform Philippine Crop Insurance Corporation about the fact of its
death and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this November
