I, ALEXANDRA ANGEL TUMONGHA, of legal age, single and a
resident of Blk. 5, Lot 8, Phase 3B, Malesido Homes, Brgy.
San Isidro, General Santos City , after being duly sworn,
hereby depose and state:
1. That I am the love child of MAY GRACE BACALSO
TUMONGHA and I was born on February 5, 1992 at
Calinan, Davao City ;
2. That my birth certificate was duly registered with the
Local Civil Registrar of Calinan, Davao City;
3. That in my SCHOOL CREDENTIALS , GOVERNMENT ID and
PRIVATE IDENTIFICATION CARD my name entered as
ALEXANDRA ANGEL BACALSO TUMONGHA instead of true and
real name ALEXANDRA ANGEL TUMONGHA
4. That ALEXANDRA ANGEL TUMONGHA and ALEXANDRA ANGEL
BACALSO TUMONGHA all refers to one and the same
person;
5. That my true and correct name is ALEXANDRA ANGEL
TUMONGHA;
6. That I executed this affidavit to attest to the truth
of the foregoing facts and to correct the said
erroneous entries.
