Affidavit of Loss Dimol Solid

Jovert P. Dimol, a warehouseman for Solid Motorcycle Distributors, Inc., filed an affidavit of loss stating that he lost the company's entry pass issued by the Cebu Port Authority. AFFIDAVIT OF LOSS

That I, JOVERT P. DIMOL, of legal age, and a resident of Tingub, Mandaue


City, after having been duly sworn to in accordance with law, hereby depose and
state:
1. That I am Warehouseman of Solid Motorcycle Distributors, Inc., a
corporation duly organized and existing under the laws of the Republic of the
Philippines;
2. That as a warehouseman, my duties, among others, is to keep important
documents of Solid Motorcycle Distributors, Inc., including the Identification Card
and Entry Pass issued to said Solid Motorcycle Distributors, Inc., from the Cebu Port
Authority;
3. That I usually keep the said Identification Card and Entry Pass issued by
the Cebu Port Authority in the cabinet inside the warehouse which right next to the
Comfort Room;
4. That sometime December, 2012, I went to the said cabinet to get the Entry
Pass because another employee of Solid Motorcycle Distributors, Inc. was about to
use it in picking up the merchandise to loaded at the Cebu Port Area;
5. That when I went to the said cabinet, I noticed that the said Entry Pass
issued by Cebu Port Authority is no longer there;
6. That I then conducted a more thorough search inside the entire warehouse,
and the entire building and in the entire compound, but my efforts proved futile as
the said Entry Pass could no longer be recovered;
7. That I now consider the said Entry Pass issued Cebu Port Authority as lost
for all legal intents and purposes;
8. That I execute this affidavit in order to attest to the truth of the foregoing
facts and for whatever purpose it may serve;

of February 2013 at Cebu City, Philippines.
