AFFIDAVIT OF LOSS

I, ARGENTINA A. GUIMEREY, Filipino, of legal age, single and a resident of


Bingag, Dauis, Bohol, Philippines, after being sworn in accordance with law, hereby depose and
say:

1. That I am a holder a certain Postal ID issued by the Philippine Postal Corporation


Tagbilaran City;

2. That the said postal id got lost around November 2018;

3. That I exerted diligent efforts to find and recover the said Postal ID, however, my
efforts proved futile as until now, its whereabouts still could not be ascertained,
hence, I considered it to be lost for all intents and purposes;

4. That said postal id is not in the hands of any third person or authority for value or
consideration or for any purpose whatsoever;

5. That I execute this affidavit voluntarily to attest to the truth of the foregoing
statements for whatever legal purpose this may serve.
