MOTION TO ALLOW ACCUSED TO UNDERGO FURTHER


MEDICAL EXAMINATION

Accused DECINO PETALCORIN y ESTILLORE, through the


undersigned counsel, and unto this Honorable Court most respectfully states that:

1. Herein accused DECINO E. PETALCORIN is complaining that he is


suffering ulcer and/or hyperacidity for how many days already and he is advised for
further medical evaluation per referral slip issued by the nurse on duty and duly
approved by Warden J/CINSP JOSE RUSYLVI ABUEVA, hereto attached as ANNEX
“1”;

2. That accused having the right to health, accused humbly requests that he
be further examine by a doctor of any government hospital;

PRAYER

WHEREFORE, premises considered, in the highest interest of justice, it is most


respectfully prayed of this Honorable Court that accused be allowed to undergo further
medical examination. Other reliefs deemed just and equitable under the premises are
likewise prayed for.
