WE, MEL ALVAREZ BOBIJES, of legal age, Filipino, married and presently
residing at Blk. 2 Lot 21 31 St. Legian 2-C Carsadang Bago 1 Imus City, Cavite, and
MICHELLE NAVARRO ALVAREZ of legal age, Filipino, single and presently residing at
Blk. 2 Lot 21 31st. Legian 2-C Imus City, Cavite, both after being duly sworn to in
accordance with law, depose and state that:

5. We are executing this affidavit to confirm and affirm the foregoing facts and
for whatever legal purpose it may serve.
