AFFIDAVIT

I, MARIA B. TAMARAY, of legal age, single, Filipino, and a resident of Tuguegarao City, Cagayan,
under oath, do hereby state:
1. That I employed MR. JOHN T. ABUAN, of legal age, male, married and resident of Annafunan,
Tumauini, Isabela as Private Caregiver to Ms. Mirasol S. Addun from August 2016 to September
2018;

2. That he was responsible in monitoring the vital signs of Ms. Addun, in feeding her meals, and in
administering her medicines on time;

3. That he also assisted my grandmother, Ms. Fidelina S. Addun, during her bath time and in her
toilet needs, he also extends help in her grooming and simple exercise every day;

4. That I observed him to be very diligent and proficient in doing the tasks I assigned to him;

5. That I can attest that he is highly trained and possesses proficient caregiving skills, and he
maintained good working relationship with me as his employer and with my family;

6. That I know all the foregoing facts based on my personal knowledge; and

7. That I voluntarily executed this affidavit without force, threat, intimidation, or coercion
whatsoever;

8. That I am executing this Affidavit to attest to the truth of the foregoing and to use the same for
whatever lawful and legal purpose it may serve.
