I. Title: Re-echoing of the Making of Quarterly Reports, Item Analysis, Form 14 and Interventions during
the Pandemic based on the District Coordinators’ Meeting for SY 2021-2022.

II. Rationale:
Forms and Reports are important to help assess the students’ needs especially during the
pandemic. Through these reports, teachers can come up with interventions that will help students
realize their full potential, even during these times.
This LAC focuses on re-orienting the teachers on the forms and planning for activities
that would cater the needs of students in competencies that are least-learned.
