I, RODYRICK GERALDE, Filipino, of legal age, and with postal


address c/o SEABIRD Ship Management, Inc., 3/F F&C Building. 2110
Sen. Gil Puyat Avenue, Pasay City, Philippines after having sworn in
accordance with the law, hereby depose and state, That - -

1. I am a Seafarer currently on board under SEABIRD Ship


Management, Inc. (“SEABIRD” for brevity);

2. SEABIRD sent me to PNTC Colleges, Inc. to undertake a


training course on a charged to agency basis;

3. I undertook training for Consolidated MARPOL 73/78


at PNTC Colleges, Inc.;

4. After I have successfully completed the aforementioned


course, PNTC Colleges, Inc. issued and delivered
Certificate of Completion dated 19 April 2011 to
SEABIRD considering that the payment of the training
fee is on a charged to agency basis;

5. I am executing this Affidavit to attest to the truth that


the Certificate of Completion dated 19 April 2011 was
issued and delivered to SEABIRD; thus, the Certificate
of Completion actually issued by PNTC Colleges, Inc.
refers to the Certificate submitted by the respondents in
the case entitled: “Nenita Azarcon vs. Hernani
Fabia” docketed as I.S. No. XV-07-INV-11l-06940
for Falsification of Private Documents pending before
Investigating Prosecutor Rachelle Likwong-Anos;
