Good day! This explanation letter is regarding to the renewal of LLLDA Discharge Permit of
_________________________________. The application for renewal for Discharge Permit should be on December
2019 but the application was delayed due to lack of requirements. One of these is the required 4 hrs seminar of
the PCO_______________ Her seminar is supposed to be in December , but since she is out of the country , she
cannot attend the said seminar. I have called To LLDA regarding this matter, but they said that the 4 hrs seminar
for PCO is needed for the renewal of Discharge Permit. I have searched for other alternatives for that and find the
recent application for the D______________________- which is prepared by _____________- Unfortunately, she
did not turnover me these things as well as the requirements for application. Honestly, it is my first time to apply
for a discharge permit so I have searched for the requirements and while doing these I am handling the
production. Because of that my attention is divided for production and PCO .The inventory for the finished
products is also needed which is to be checked by the Production head. Last December, there are many orders that
needs to be encoded like ________________--, needs OF and I need to put batch nos.so that the QA can make the
QC passed because most of the ordered products are for delivery. The new ______________________-- first day is
last December 26, 2019. I have taught him how to make the SMR for and . We have applied for the Discharge
Permit last January 8, 2020 and assessed the application on the same day. The assessor tells us to pay the
assessment on that day . We came back to ___________- around 12:30 pm at that day , but on the way I have told
Sir Fred regarding the payment as well as the consequences if the Discharge Permit was not paid on that day,
another 5, 000 will be charged. We have made a request letter for payment and told these to _____________- ,
and she said the signatories were all on leave and there is no cash like that here. Then, I have relayed this payment
to Ms. Eva and she said me to forward the request letter for payment because they have check there to be
encashed. Then, ___________--ame to eagle to get the payment but unfortunately the BPI bank was offline and
Jason waited until 4:30 unfortunately he don’t get the payment. I also called____________- if they have a bank
account but they said that they not allow deposits thru bank because there will be no official receipt. The llda is
open only up to 4:30 pm and in that case the payment will not be given on the day. As an alternative plan, we
made a letter citing the reason why we don’t have pay the fees yesterday and asking for consideration to waive the
additional charge of 5000 if paid the other day. Jason came to LLDA early in the morning to ask for consideration
and consider the first assessment amounting to 18,003.06 , Unfortunately, the assessor did not give consideration
and said the contract should be followed , a new assessment was given which has the additional charge of 5000.
Jason paid first the additional charge of 5000 and will ask for reimbursement. The Discharge Permit is for release
within 15 working days provided that all requirements and fees have been satisfactorily complied with.
