Dear Dr Brown,

Re: Mr David Taylor, 38 years old

I am writing to refer Mr David who is been diagnosed with damaged medial cartilage of left knee joint.
Therefore needs an operation to remove damaged cartilage in order to prevent future osteoporosis.

Mr Taylor is an active person and enjoy playing sports. Please note he has a history of twisting of the left
knee joint 6 months ago for which he took painkillers to subside the pain and swelling.

On 12/08/10, Mr Taylor presented with complaints of left knee joint pain and swelling. In addition, he
had difficulty in straightening the leg. On examination, he was limping and a tender spot on the medial
aspect of the joint was noted. Consequently, X ray was ordered and a pain killer was prescribed
accordingly . However, on his subsequent visit in addition to his previous complaints he mentioned of
intermittent attacks of pain and swelling of the left knee joint. In addition with impaired passive and
active motion. Hence, an MRI was ordered.

On today’s review Mr David, mentioned that his joint pain was decreased but swelling was still present.
Furth more, his MRI report revealed damaged medial cartilage of left knee joint.

In view of above, an appointment on 21/11/10 has been arranged for further treatment and
management.
