Re: Mr David Taylor
Aged: 38 years
I am writing to you concerning Mr Taylor, a 38-years old married man with three children and
runs his own business as a landscape gardener seen at my facility with the diagnosis of injury of
the medial cartilage for further management at your facility.
Mr Taylor has a past medical history of left inguinal hernia operation in 2008. He is active and
enjoys sports, smokes as well as drinks alcohol.
On 12/08/10, Mr Taylor came to the hospital complaining of knee joint, swelling and difficulty in
strengthening the leg. He further reported twisting the left knee joint about 6 months ago in a
game of tennis. He finds the injury preventing him from working productively and is worried as
he needs regular income to support the family as well as home repayments. Upon assessments
, he had a limp, the left knee was slightly swollen, there was a tender spot on the medial aspect
of the joint and no effusion. basic observations taken were normal, an x- ray of the knee was
taken and he was put on voltarin 50mg twice daily. He was advised to reduce smoking and
come for review of no improvement.
Mr Taylor reported back to the hospital on 25/08/10 with the same symptoms from the previous
visit. An MRI of the knee was ordered and he was asked to continue with the same dosage of
her previous visit medication then come for a review after a week with the investigation. He
came back to the hospital with the MRI report on 07/11/10. The report showed damage of the
medial cartilage, limp was still present and he had increased weight by 5kg.
In view of the above, I would be very grateful if you could see Mr Taylor and remove the
damaged cartilage in order to prevent future osteoporosis. An appointment for him has been
scheduled with your receptionist at 8am on 21/11/10.
