Dear Dr. Brown,
Re: Mr. David Taylor, 38 years old
Thank you for seeing Mr. David Taylor, a landscape gardener, who has damaged medical cartilage
of the left knee. I am referring him for your specialist assessment and consideration for a cartilage
removal.
Mr. Taylor initially presented on 12th of August with tenderness and swollen of the left knee
following a tennis game injury. Examinations revealed tender point on the medial aspect of the left
knee joint. He was commenced on Voltaire 50 mg twice a day and was advised to reduced
smoking.
On a review on 25th of August, Mr. Taylor complained intermittent pain and swelling of his left
knee joint. Therefore, his left knee joint remained swollen and developed limited range of power
on passive and active movements. As a result, the MRI was performed to con rmed the potential
medical cartilage injury.
Today, Mr. Taylor still has limping gait and expressed his concern regarding this issues. He is now
desperate and his weight increased by 5 kg. The MRI reported the damage of this left knee
medical cartilage.
Mr.Taylor is generally healthy and has no personal injury insurance. He is a smoker.
In light of the above, It would be greatly appreciated if you could provide additional evaluations
and consideration for a cartilage removal in order to prevent future osteoporosis. Please note that
I already contacted your receptionist and made an appointment for him at 8am on 21/11/10.
If you have any questions, please do not hesitate to contact me.
Yours sincerely,
Doctor
