Service Providers Evalution Form

This document provides a questionnaire to assess the performance of a service provider based on their adherence to contract requirements. The questionnaire contains four criteria: (1) Quality of Work/Service, (2) Timeliness of Performance, (3) Business Relation, and (4) Stability and Resources. conformity to all requirements as stated and agreed upon the Contract. We have Stored essential
questions into four (4) different criteria’s that will be filled up by the End-User/Requisitioning Office.
