Check whether the staff on duty is alert and in prescribed uniform.
2. Match the entries in TSR and charge book regarding Engg. Block, Power block
and disconnection of gears. The entries in TSR of these blocks are done in red
ink.
3. Check the Panel/RRI/VDU counter numbers and match it with the relevant
register. Summary of counter Nos. is being made.
4. The caution order register was maintained properly and being forwarded every
Monday.
5. All safety literature and rule books are available and updated. Staff is well
aware of SWR. /Safety circular/Extant rules with assurance of staff.
6. The Signal Failure register (SFR) register was checked, the last failure was
on___________ for_________________________________ from ______to
_______Hrs. Summary is being prepared gear wise and department wise.
7. Check signal failure register and see that the S&T failure are not unreasonable
high and are promptly attended, highlight repeated failure if any.
8. The relay room was last opened on dt. __________from ______hrs to ______hrs.
for __________. Proper procedure for opening and closing of relay room was
followed. Signal control serial No. for opening and closing is recorded. Summary
is being prepared regularly.
9. Last Disconnection of S&T gear was allowed on dt ___________ of _________
_________from________to______hours. Summary is being prepared regularly.
10. Emergency S&T key box was found paper sealed by S&T supervisor and locked
by SM. Relay room is Double locked.
11. Ambush check after opening and closing of relay room whether the pop up
message is received by concerned staff including TI, SS and SI control. Also
check if Signal control warns staff such as Sr. DSTE-CO, RPF control, SS and
SSE-SIG-in charge in case of unauthorised opening of relay room.
12. Match the timing of failure recorded in S&T failure and memo issued to S&T
staff.
13. The main supply switch which is to be turned off in the event of Fire in the relay
room is specifically marked or labelled as per JPO.
14. The SS conducts fortnight inspection. The last inspection was carried out on
______. Check the quality of inspection and counsel SS if required.
15. Check the monthly safety inspection of TI. The last inspection was carried out on
________with ________irregularities, check whether the irregularities were
complied or not. Check the quality of inspection.
16. Check the Counselling register and note the last counselling done by TI- on
_______. Test the knowledge of staff regarding the subjects counselled by TI.
17. Anemometer is in working order where ever is available.
18. Check the availability of safety posters in station office.
19. Check joint points and crossing register (JPC) and see whether the inspection is
conducted quarterly as per schedule and deficiencies are complied within 15 days
of inspection.
20. Monthly Accident Prevention meeting is being carried out by SS/SM with
number of staff attended. Test the knowledge of staff regarding the subjects
counselled in Accident prevention meeting.
21. Check that Quarterly Fire mock drill is being conducted together with the
assurance of ______ staff. Check the knowledge of staff regarding operation of
fire extinguisher.
22. The First aid box was refilled on ______ and all the items are complete and
within expiry date.
23. The detonators available are matching with the manufacturing and expiry date on
label and printed on the detonators and are found within the expiry date.
24. All safety equipment are available as per app “E” of SWR and are in good
condition.
25. The Gradation register was checked and found that all group C and group D
staffs were graded properly and acknowledgement to be taken from staff.
26. The monthly crank handle demonstration is being done every month by SS/SM
and name of staff attended is recorded. Match the date of paper seal and last
crank handle usage from the register.
27. Check that a safety booklet issued by safety organization is available of level
crossing gate, securing of stabled, precaution regarding OHE safety and check list
of Station inspection
28. Station Working Rule (SWR) (English/ Hindi) are available, with latest
Correction Slips. SWR Diagram & OHE diagram are matching with the Actual
Lay out of the yard. Knowledge of staff with regard to SWR is adequate.
29. Diesel generator set is in working order wherever available and is being tested as
per rule.
30. Sufficient button collars are available wherever is applicable and it is placed on
block line.
31. Joint correspondence test of concerned points by S&T staff and SM has been
carried out before accepting the reconnection memo.
32. Check Securing of stabled loads by safety chains, wooden wedges and applying
hand brakes on either side of load. Check the stable load register for loco
pilot/train manager/ operator’s signature after stabling. Copy of GDR is available
in case of train stabled for more than 24 hours.
33. Check the Bio data register and see that the staffs are not overdue for PME,
Refresher and OHE competency.
34. Charge book and TSR is being maintained without any overwriting. Charge book
is serially numbered.
35. Emergency telephone Nos. board is available and updated. Check any random
number and see if it’s in working order. Also check that the emergency hospital
numbers are saved in the personal mobile phones of station masters.
36. The officer’s last inspection was carried out on _________with _______
irregularities, check whether the irregularities were compiled or not.
37. Check that Reversal of points is done immediately after complete arrival of train.
38. Check that sliding boom is being tested every Monday by Traffic LC gateman
and it is being recorded by station master in charge book or TSR.
39. Inspection of WLI is as per schedule.
