sworn in accordance with law hereby depose and state THAT:
1. We are the owners of the houses and parcels of land located at___________________;
2. We hereby signify our consent freely, willingly and voluntarily to allow Ms.
Madona M. Orillaneda to construct electric connection line in favor of the
adjacent houses that will transverse our houses;
3. As such, We are executing this Affidavit to attest to the truth of the foregoing
facts as to our undertakings to give consent to Madona M. Orillanedato
support her construction of electric lines.

We, GREGORIA M. UMANDAP, JAYSHON M. UMANDAP, ALBERT M. UMANDAP,
MICHAEL M. UMANDAP, EMELDA U. NATANAUAN, CHONA U. QUEZON, GRETCHEN U.
CARAAN and DIANNE M. UMANDAP, all Filipinos, of legal ages, residing at Barangay As-is,
Laurel, Batangas, Philippines, after having been sworn in accordance with law hereby depose
and state THAT:
1. We are the heirs of EUSEBIO M. UMANDAP who died on August 02, 2015 and one of
the owners /holders of a joint bank account no. 240810064496 with Philippine
National Bank Lipa City Branch together with NEPOMUCENO M. UMANDAP;
2. The said joint account is used for the purpose of paying the amortization due on a
parcel of land owned by our family which was mortgaged to the above said bank with
the remaining balance payable for 9 months now ;
3. For not to cause problems with the bank transactions and by reason of death of the
said deceased, we, hereby give our consent, freely, voluntarily and willingly to
allow/authorize NEPOMUCENO M. UMANDAP to transact, process, execute and sign in
our behalf any/all documents pertaining to the said bank account until the amortization
has been fully paid.;
4. As such, We are executing this Affidavit to attest to the truth of the foregoing facts as to
our undertakings to give consent to NEPOMUCENO M. UMANDAP and support all his
legal transactions with the bank.
