DEFECT REPORTS
(CAR PART 43.02.4)

1. SCHEDULED MAINTENANCE

Defects that are identified during any maintenance or inspection of an Aircraft or component as a result
of a work pack/task card, shall be recorded on a TFU, and will form part of the issued work pack.
This shall be depicted on an Index of the particular work pack.

2. PROCEDURE FOR DEFERMENT AND RECORDING OF DEFECTS. (SA-


CATS-GMR, PART 43.02.3 & AIC 60.14 06-07-07 REQUIREMENT ‘J’)

2.1.Deferred Defects Policy

All aircraft flight folios are to be actioned at all stations prior to continuation of flights.
Transfer of items to the “Deferred Defects Check List” are only to be made when rectification action
cannot be accomplished due to technical reasons, lack of spares or because the flight schedule is
to be maintained, under consideration of the MEL and DDPM/CDL. Open reports are transferred
solely by the responsible Manager of his authorized designates in the following manner: DEFER
DEFECT SHEETS

2.1.1. Confirmation of the defect and the reason for deferment shall be entered together with the
MEL and DDPM/CDL reference and category type plus expiry date in the action block
of the relevant flight folio. The entry must start with the wording “DEFERRED DEFECT”.
Refer to check item if arising from hanger maintenance.

2.1.2. Remove original (white) slip with the yellow copies. Place a coloured sticker next to the
affected system i.e. switch, gauge, light etc. On the sticker write the MEL/DDPM/CDL
Authority
number and flight folio reference of the inoperative condition. This will alert the crew to the
Deferred item.

2.1.3. Enter the Flight sequential number the MEL reference and category and expiry date
plus Limitations as well as a short description of the defect onto the deferred defect check
list. Stamp for authorization (Only designates may stamp for authorization).

The following persons shall be advised of any deferred defects prior to aircraft dispatch:

A. NAC Person Responsible Aircraft.

B. Aircrew informed prior to A/C Acceptance

2.1.4. The Contracted AMO to capture and file defer defect from the flight folio.

All are responsible for reworking of deferred defects in a timely manner. On subsequent
clearance of deferred defect the following action shall be taken by the relevant Crew
Chief, Flight Line or Hanger Maintenance.

2.1.5. On the next available blank Flight Folio, cross refer to the original deferred defect
slip in the cross reference block, entering the original Flight Folio sequence number.

2.1.6. The Flight Folio sequence number along with the date repaired and authority stamp shall
entered onto the deferred check list under the appropriate column.

2.1.7. Remove and destroy the alerting sticker.

3. CONTROL DEFERRED DEFECTS.

All defects should, where practicable, be rectified as soon as they are entered in the Flight Folio. However,
due to operational reasons, certain defects may be deferred until a later date, if time, spares or resources
prevent their immediate rectification. In this case, defects shall be deferred as per Minimum Equipment List.
Aircraft defects are classified as “A”, “B”, “C” or “D” defects – (Minimum Equipment List MEL refers.)

If a defect cannot be actioned/rectified in the specified time frame a one time extension of the rectification
interval CAT B or C for the same duration as specified in the MEL may be granted. This MEL extension
application will be recorded and retained on record.

3.1Category “A”

“A” defects usually require immediate rectification. They are normally of an airworthiness nature
outside the scope of the Minimum Equipment List or the Structural Repair Manual for example and
are not capable of being carried forward. Where practicable be rectified immediately, if not, it must be
rectified at next Base stop.

3.2 Category “B”

“B” defects should, where practicable, be rectified immediately. They may be of an airworthiness
nature; however, they are less serious than “A” defects and can be identified within the Minimum
Equipment List, Aircraft Maintenance Manual, or Structural Repair Manual, they may be deferred for
3 consecutive calendar days (72 hours). Such deferments must be made in accordance with
approved data, i.e. Flight Manual or Minimum Equipment List. Rectification of “B” defects should
take place at the earliest opportunity.

3.3 Category “C”

“C” defects are defects with no airworthiness implications or limitations via the Minimum Equipment
List, Maintenance Manual or Structural Repair Manual. “C” defects may be deferred until
rectification is practicable. Items in this category shall be repaired within ten (10) consecutive
calendar days (240 hours)excluding the day the malfunction was recorded in the Flight Folio.

3.4 Category “D”

“D” Defects are defects with no Airworthiness implications or limitations via the Minimum Equipment List,
Maintenance Manual or Structural Repair Manual. “D” Defects may be deferred until rectification is
practicable. Items in this category shall be repaired within one hundred and twenty
(120) consecutive calendar days (2880 hours) excluding the day the malfunction was recorded in
the Flight Folio.

4. ANALYSIS

Flight Folio pages must be examined at regular intervals by the Contracted AMO to provide information
concerning defects occurring, Pilot's reports, maintenance actions and defects of a repetitive nature.

Maintenance input records (work-packs) will also be reviewed for significant findings by the Contracted AMO
which may have airworthiness or operational implications.

The Person Responsible Aircraft will assess the findings as necessary and has to agree to any action
required before implementation. Implementation may take the form of a Maintenance Programme
amendment or modification action.

5. LIAISON WITH MANUFACTURERS

The Person Responsible Aircraft shall ensure that all major defects that can adversely effect the
airworthiness of the Aircraft, be communicated to the Manufacturer.

6. NON DEFERRABLE DEFECTS POLICY

Non deferrable defects must be repaired before further flight. If defects are not covered by approved data the
Type Certificate Holder has to be involved. Alternately an exemption application is to be submitted to the SA
CAA.

7. REPETITIVE DEFECTS

The Flight Folio is monitored by the Person Responsible Aircraft to identify these defects as and
when they arise. Remedial action will be taken by the Contracted AMO in consultation with the Person
Responsible Aircraft.
