City of Davao, Island of Mindanao, containing an area of ONE
HUNDRED AND THIRTY THREE (133) square meters, more or
less, because I am now 18 years old, attached herewith is my
Certificate of Live Birth referred to as “ANNEX A” ;

2. I am executing this affidavit to attest to the truth and to inform the


proper authorities concerned and for whatever legal purposes this
may serve.
