IMPORTANT NOTE: Prior to undertaking any inspection, ensure that you have planned the task accordingly. Remove all unnecessary tools or personal equipment from pockets. Take a tool bag or
other secure container with you to carry any loose items you remove from the worksite. Ensure that these inspection sheets and your pen or pencil is secured to prevent them falling from height.
Ensure you have inspected all personal fall protection equipment and that you have notified other that you are performing an inspection.
To assist you further, we recommend that you review all dropped object incident reports and observations from operations so that you may focus on specific areas of potential for dropped objects
as appropriate.
REMEMBER TO CHECK ALL PAD EYES AND LIFTING EQUIPMENT FOR CERTIFICATION (ie CURRENT COLOUR CODE)
GENERAL NOTE: REPORT ALL SIGNS OF COLLISION DAMAGE OR EXCESSIVE CORROSION ON ALL STRUCTURE AND EQUIPMENT
COMMON FAILURES: Constant vibration, temperature change and other external environmental factors contribute to loss of tension in bolts, screws, clips and brackets. Poor design of mountings
can lead to galvanic corrosion on guardrails (e.g. poorly selected U-bolts or home-made clamps). General corrosion of structure can lead to loose material and weakening of mounts. Accidental
collision can lead to weakening of fixings
