AFFIDAVIT

That we, PCpl Ricardo Yabo and PCpl Manuel Leonen, bona
fide members of the PHILIPPINE NATIONAL POLICE assigned at
Ozamiz City Police Station after having been duly sworn to in
accordance with law do hereby depose and say the following:

1. That around 4:40 in the afternoon of July 22, 2024, we


responded to a call for police assistance at P-1, Barangay
Maningcol, Ozamiz City, Misamis Occidental as per information
received from the Duty Desk Officer PCpl Randy Ricaforte that
there was a shooting incident that transpired on that particular
place.

2. That upon our arrival at the reported place of incident,


a person named Marisol Chiu, who positively identified the victim
and gave vital information of the suspect and his possible
whereabouts and that I, PCpl Ricardo Yabo, called the Ozamiz
City Police Station and PLT Diosdado Dalisay also informed us
about the recent call through radio transceiver from Hon.
Richard Manzano, Barangay Chairman of Barangay Maningcol,
Ozamiz City.

3. That we, PCpl Ricardo Yabo and PCpl Manuel Leonen,


immediately proceeded and conducted hot pursuit operation at
the national highway in Barangay Maningcol, Ozamiz City to the
direction where the suspect had reportedly fled from the incident
while riding a motorcycle.

4. That I, PCpl Manuel Leonen, was equipped with Body-


Worn Camera (BWC) in compliance with the directives issued by
the Supreme Court.

5. That upon proceeding on the highway, I, PCpl Ricardo


Yabo, received a call from PLT Diosdado Dalisay that the suspect
John Michael P. Madrigal had voluntarily surrendered at the
Ozamiz City Police Station.

6. That I, PCpl Manuel Leonen, assisted PCpl Ricardo


Yabo in responding to the reported place of incident.
7. That we are executing this affidavit freely and
voluntarily purposely to support the filing of the aforesaid case
against the above-mentioned suspect.
