Pursuant to paragraph six (6) 389 of Republic Act 7160, otherwise known as the local code
of Government Code of 1991, you are hereby appointed as Barangay Health worker(BHW),
of this Barangay effective upon taking your oath.

But upon your negligence of duties and responsibilities and to your consecutive absences
without valid reason, You are now terminated as barangay health worker in this barangay
upon received this noticed of termination effective this date stated, October 10, 2022.
