The undersigned are student-researchers at Jacobo Z. Gonzales Memorial
National High School who are currently conducting a Science Investigatory
Project (SIP), entitled “PROJECT A.I.D (ACCESS IN DANGER): IMMEDIATE
MEDICAL RESPONSE MOBILE APPLICATION”. The main objective of this
study is focused on the development of a localized and contextualized
learning module towards higher conceptual understanding in Research IV
among grade 10 students.
In connection with this, I would like to request permission from your office to
conduct my research among 20 Grade 10 students at our school. Rest
assured that the data gathered will be used solely for research purposes and
will be kept with the utmost confidentiality.
