AFFIDAVIT OF COMPLAINT

I, JHOANA MICHELLE RAPADA Y MACATUNOG, 24 years old, Filipino, single,


business woman, and a resident of Barangay 30B Santa Maria, Laoag City, Ilocos
Norte, after having been duly sworn to in accordance with law, do hereby depose and
state:

1. That I am the friend of Zedrick Corpuz y Erice, 35 years old, Filipino, married,
farmer, and a resident of Brgy. 30B Santa Maria, Laoag City, Ilocos Norte
while he was alive;

2. That on January 30, 2025, at approximately 5:30 in the afternoon, I was


passing by Zedrick’s house after going to grocery store, when I suddenly saw
the body of Zedrick lying on the ground, covered in blood, lifeless;

3. That I immediately rushed him to the nearest hospital (Governor Roque Ablan
Sr. Memorial Hospital) where I was informed that my friend had been
pronounced Dead on Arrival (DOA);

4. That according to the police investigation, my friend and the suspect had a
drinking spree that caused them to have a heated altercation over the land
dispute, he struck my friend using his “BOLO”;

5. That I am executing this sworn statement to attest to the facts stated above
and to file a formal complaint against DENMARK GOMEZ Y AGREGADO for
the crime of Homicide under violation of Article 249 of the Revised Penal
Code;

6. That I am executing this sworn statement freely and voluntarily without any
force, threat, or intimidation from any person or entity.

IN WITNESS WHEREOF, I have hereunto set my hand this 2 nd day of February 2025 at Laoag
