Lincosamide: Clindamycin
Kleenex = Clindamycin
50 cent Kleenex tissues = binds to 50S subunit of bacterial ribosome - blocks/interferes
with peptide transfer (ie translocation) → decreased protein synthesis
Holding breath = used to treat anaerobic infection (above the diaphragm)- eg aspiration
pneumonia, lung abscess, oral infections
Remember Clindamycin ABOVE the diaphragm, Metronidazole BELOW the diaphragm
for anaerobes
Diarrhea like smelly mud = causes C. difficile/ pseudomembranous colitis
Diffuser = difficile

Oxazolidinones: Linezolid

Side effects:

3) vomiting = GI motility issues (Hypertrophic pyloric stenosis in infants and laxative
effect esp with Erythromycin) - imagine someone holding a cocktail with an olive in it
(like olive shaped mass)- it is a motilin receptor agonist
4) twisted streamers = Torsades de Pointes - QT prolongation
Clarithromycin- is embryotoxic
Clarithromycin and Erythromycin inhibit CYtochrome P-450 enzymes in liver → can
cause drug interactions:
Increases theophylline and oral anticoagulants (Warfarin) serum levels by decreasing
liver clearance due to CYP3A4 inhibition
(CYP3A4 is a type of enzyme under CYP450)
Azithromycin doesnt affect CYP enzyme so its safe to give in pts with liver problems.
Polymyxins
Pancake mix, mixer = Polymyxins
Colander = includes Colistin (Polymyxin E)
Tearing open plastic wrap = disrupts cell membranes (Cation polypeptides bind to
phospholipids on cell membrane of gram ⊝ bacteria and increase membrane
permeability)
Uses: not used first line for anything - used as last resort for severe MDR gram -ve
bacteria (K. pneum, E.coli, P. aeruginosa) - cus it has so many side effects
Trimethoprim:
Panda sitting on a foal = inhibits folate synthesis (inhibits dihydrofolate reductase)

veggies have fallen to the floor (decreased folate) and a huge stalk of bananas are

Rifamycins: Rifampin, rifabutin, rifapentine

Isoniazid (INH)

Popping tube = TB ; impairs synthesis of mycolic acid ie it treats Tuberculosis

gene)
KitKAT next to peroxide = INH resistance arises due to mutations in katG gene

ie have low level of acetylase
Acetylase metabolizes the drug in liver - so different INH half-lives in fast vs slow
acetylators- and slow acetylators have more side effects cus drug stays in body for longer
Side effects:

neuropathy, sideroblastic anemia)- so we have to administer INH with Vit B6
Beer bong = inhibits sips; inhibits CYP enzymes
Wolf like husky = causes drug induced lupus (lupus like reaction: discoid rash,
arthralgias) but this is reversible
Liver shaped lever = hepatotoxicity (increased risk with alcohol and older age)

Pyrazinamide

Can affect absorption, bioavailability, or urinary excretion of other drugs by altering
gastric and urinary pH or by delaying gastric emptying. All can cause hypokalemia.
Octreotide
Octagon sign = Octreotide
Stop sign = somatostatin analogue
Monster truck = giant; treats acromegaly/gigantism (inhibits GH release)
Large veins in neck = treats esophageal varices - it inhibits glucagon production (a
splanchnic vasodilator). In advanced liver disease, portal vein gets blocked, leading to
portocaval shunting such as esophageal varices. Somatostatin causes splanchnic
vasoconstriction and thus less blood flowing through esophageal varices.
Leaking brown oil = used in secretory diarrhea (ie diarrhea caused by excess hormones
eg VIPoma, carcinoid syndrome)
Stones falling = increases risk of gallstones (cus it inhibits CCK release)
Diphenoxylate & Loperamide
Dyed socks = Diphenoxylate
