A RESOLUTION IN SUPPORT OF THE PUROKALUSUGAN PROGRAM OF THE DEPARTMENT OF HEALTH


(DOH)

WHEREAS, the PuroKalusugan program of the Department of Health (DOH) aims to promote health and
well-being in communities through strategic approach, comprehensive and responsive, people centered
health initiatives, education and services;

WHEREAS, Barangay San Jose recognizes the significant benefits of supporting public health programs
that contribute to the overall development and well-being of its residents;

WHEREAS, the PuroKalusugan program focuses on preventive healthcare, disease prevention, and health
promotion, targeting the following eight priority health outcomes:

1. Immunization

2. Nutrition

3. Maternal Health

4. Water, Sanitation and Hygiene (WASH)

5. Tuberculosis and HIV/AIDS Management and Control

6. Road Safety

7. Non-communicable Diseases (NCD), including Hypertension, Diabetes Control, and Mental


Health

8. Cancer Prevention and Control, and Hepatitis B Management

WHEREAS, the PuroKalusugan Health Team shall be composed of:

1. At least one (1) nurse or midwife from the National Health Workforce Support System (NHWSS);

2. DOH-deployed personnel at the LGU level, hired by other DOH Offices such as Center for Health
Development (CHD) and Disease Prevention and Control Bureau;

3. The assigned nurse and midwife supervisor from the LGU; and

4. Barangay Health Workers and Barangay Nutrition Scholars;

5. Other identified work force.

WEEKLY schedule of Purok visits

To ensure comprehensive health service coverage within Barangay San Jose, a purok visitation or
house to house visit will be conducted by the puroKalusugan Health Team within the barangay, giving
prioritization to the most vulnerable group or the underserved and unserved constituents of the
barangay.

WHEREAS, the Barangay Council of San Jose acknowledges the importance of collaboration and active
participation in the implementation of health programs to ensure their success and sustainability;

WHEREAS, the following legal frameworks and policies support the PuroKalusugan program:

1. Republic Act No. 11223 - Universal Health Care Act


2. Republic Act No. 11148 - Kalusugan at Nutrisyon ng Mag-Nanay Act

3. Republic Act No. 10354 - Responsible Parenthood and Reproductive Health Act of 2012 (RPRH
Law)

4. Republic Act No. 11332 - Mandatory Reporting of Notifiable Diseases and Health Events of Public
Health Concern Act

5. Presidential Decree No. 856 - Code on Sanitation of the Philippines

6. Republic Act No. 8504 - Philippine AIDS Prevention and Control Act of 1998

7. DOH Department Memorandum No. 2025-0024

8. DOH Department Memorandum No. 2025-0011

NOW, THEREFORE, on motion of Barangay Council Member ______________, duly seconded by


Barangay Council Member ______________, be it:

RESOLVED, as it is hereby resolved, to fully support the PuroKalusugan program of the Department of
Health (DOH) and to actively participate in its initiatives, activities, and campaigns to promote health and
well-being in Barangay San Martin;

RESOLVED FURTHER, that the Barangay Council of San Jose, through its Barangay Local Health Board,
shall allocate appropriate resources and provide necessary assistance to ensure the effective
implementation of the PuroKalusugan program within the community;

RESOLVED FURTHER, that the proposed budget and other resources for the implementation of the
PuroKalusugan program are as follows:

Proposed Budget and Resources:

1. Health Education and Promotion: PHP ___________ allocated for educational materials,
community events, and health promotion activities.

2. Medical Supplies and Equipment: PHP _________ allocated for purchasing medical supplies,
immunization materials, and hygiene kits.

3. Personnel Training and Development: PHP ___________ allocated for training and capacity-
building programs for Barangay Health Workers, Nutrition Scholars, and other health personnel.

4. Infrastructure and Facilities: PHP _________ allocated for the maintenance and improvement of
health facilities and infrastructure within the barangay.

5. Emergency Response and Contingency Fund: PHP _____________ allocated for emergency
medical response and unforeseen health-related contingencies.

RESOLVED FURTHER, that the planning and implementation of the PuroKalusugan program will follow
these steps:

Planning and Implementation Steps:

1. Needs Assessment: Conduct a thorough assessment of the health needs and priorities of the
community through surveys, focus group discussions, and consultations with stakeholders.

2. Resource Mobilization: Identify and secure resources, including funding, materials, and
personnel, from local government units (LGUs), non-governmental organizations (NGOs), and
other relevant agencies.

3. Capacity Building: Provide training and development programs for health personnel, including
Barangay Health Workers and Nutrition Scholars, to enhance their skills and knowledge in
delivering health services.
4. Program Development: Design and develop health programs and initiatives based on the needs
assessment, focusing on the eight priority health outcomes.

5. Community Engagement: Engage the community through awareness campaigns, workshops,


and meetings to ensure their active participation and support for the program.

6. Implementation: Roll out the health programs and initiatives in a phased manner, ensuring
proper coordination and collaboration among all stakeholders.

7. Monitoring and Evaluation: Establish a monitoring and evaluation system to track the progress
and impact of the program, identify challenges, and make necessary adjustments.

8. Sustainability: Develop strategies

RESOLVED FINALLY, that copies of this resolution be furnished to the Department of Health
(DOH) and other relevant agencies for their information and appropriate action.
