RESOLUTION NO. 5-2025
“A RESOLUTION INTITUTIONALIZING THE PUROKALUSUGAN PROGRAM AS A COMMUNITY-DRIVEN
DEVELOPMENT PLATFORM FOR THE WHOLE-OF-SOCIETY APPROACH IN THE DELIVERY OF BASIC
HEALTH SERVICES AT BARANGAY DANSULAO ”
WHEREAS, Brgy. Dansulao declares that it is the policy of the LGU Government to recognize the
importance of purok of organizations as community-based convergence points that enhance the
effectiveness and efficiency of delivering basic services for the general welfare;
WHEREAS, health is fundamental to sustainable economic growth, and to improved standards of
living and when health is at risk, everything is at risk including economic and social stability and
security;
WHEREAS, the Universal Health Care Law (RA 11223) aims to enhance access for all individuals,
regardless of location, to essential health services without financial hardship;
WHEREAS, Puroks serve as catalysts for change, facilitating whole-of-government transformations
by cascading objectives to the grassroots level, empowering communities, and ensuring that health
and other basic government services are not only accessible but also tailored to the unique needs of
each community;
WHEREAS, the PuroKalusugan Program aims to translate health interventions-including provision,
promotion, prevention, and protection-and non-health interventions focusing on social protection
into grassroots actions, with Puroks serving as the frontline for delivering services and monitoring
implementation; and
WHEREAS, in addition to the 8 Priority Health Outcomes required by the Department of Health
(DOH) for “Purok Kalusugan” (Tuberculosis, Immunization, Water Sanitation and Hyglene Road
safety, Hypertension and Diabetes, Cancer, Nutrition, and Maternal Health), the City Government is
also mandated to address disease surveillance and response, along with other basic government
services (social protection and welfare, livelihood and employment, disaster preparedness), to meet
the identified needs of the puroks and the community in general;
NOW THEREFORE, on motion of Barangay Kagawad Hon. Ricky S. Pacquing , duly seconded by
August Body , be it;
RESOLVED, as it is hereby resolved, to institutionalize the PuroKalusugan Program as a Community-
Driven Development Platform for the whole-of-society approach in the delivery of basic health
services at Barangay Dansulao.
SECTION 1. OBJECTIVES OF “PUROK KALUSUGAN PROGRAM”
1.1 Achieve effective urban governance by fostering collaboration and decision-making
processes that generates collective solutions. This will be accomplished through co-
creating practices and institutional engagement, aligned with whole-of-government and
whole-of-society approaches, by actively interacting with grassroots stakeholders
through participatory governance and consensus-building.
1.2 Develop resilient and sustainable communities (Puroks) by promoting active
engagement with both health and non-health sectors, rooted in the spirit of the
Bayanihan concept, thereby strengthening the capacity of communities to adapt and
thrive.
1.3 Empower individuals and communities (Puroks) to take ownership of their health and
the development of their communities, fostering a culture of self-reliance and proactive
involvement in local governance and community-building efforts.
SECTION 2. IMPLEMENTATION OF THE “PUROKALUSUGAN, BRGY. DANSULAO OF THE
MUNICIPALITY OF MALANGAS
2.1 Steering Committee: A Steering Committee is hereby established to oversee the overall
management and implementation of the “Purok kalusugan” programs. This committee will be
chaired by the Brgy. Captain and will include the following as members:
a. RURAL HEALTH MIDWIFE- Necelyn Teves
b. NDP- Clarisse P. Salomon
c. KAGAWAD ON HEALTH- Hon. Ricky S. Pacquing
d. BHWs- Jocelyn S. Pegarido , Elena Daraman , Teresita Pegarido, Anabell Velasco,
Myrna Diego, Zenaida Velasco, Antonieta Jimenez, Miraflor Castor, Clarita Avelino
Erlenda Gonora.
e. BNS- Gina Escalon, Victoria Pacquing, Marilyn Intol ,Glenda Pacquing
f. PUROK PRESIDENT( Sitio Tapucan/Upper Dansulao/Lower Dansulao)
2.2 Partner Agencies: The program shall actively encourage the participation of various
government agencies, including but not limited to the following:
a. Department of Health
b. Department of Social Welfare and Development
c. Department of Education
2.3 Non-Government Organization and Private Sector Participation. Non-Government
Organizations and private sector entities whose interest is to contribute in “kalusugan” are enjoined
to engage in the implementation of the programs by sharing their available resource. Private firms
through their corporate social responsibility are welcome to provide support.
SECTION 3. TASKS OF CONCERNED OFFICES
3.1. All heads of offices are directed to continue leading and innovating public services, with
a focus on providing social protection to the most vulnerable groups. They shall also work to
enhance access to these services for all residents,, ensuring inclusivity and responsiveness to
community needs.
3.2. Heads of partner agencies are strongly encouraged to actively participate in the
activities of the “Purok KalusuganProgram, particularly in the delivery of essential government
services. Their involvement is crucial to the success and to ensure that services reach the grassroots
level effectively
SECTION 4. GUIDELINES FOR THE “PUROK KALUSUGAN” PROGRAM AND DELIVERY OF BASIC HEALTH
SERVICES
4.1 The Barangay, in collaboration with partner agencies, shall prioritize the delivery of
health services to all Puroks, irrespective of their location. This ensures that all communities, no
matter how remote receive the support and services they need.
4.2 The Core Lead offices shall work closely with all participating agencies and entitles to
identify and organize the specific government service to be delivered through the program. The
scope of the program is organized, but not limited to the following categories:
a. 8 Priority Health Outcomes
b. E-Konsulta Services
c. Social Protection and Disaster Preparedness
4.3 The designated lead agency shall collaborate with Purok Officials to identify the families
and individuals who should be prioritized for the delivery of health services, based on their specific
needs.
4.4 The designated lead agency shall regularly monitor and evaluate the implementation of
the “PuroKalusugan” Program, as well as the delivery of other health services, to ensure its
effectiveness in achieving the desired outcomes.
SECTION 5. ROLES OF BARANGAYS AND PUROKS
5.1 Barangays. To advance good governance at the barangay level, residents of the
barangays, as beneficiaries of this program are encouraged to actively participate in the program
implementation. This effort shall be spearhead by the barangay officials, in accordance with their
mandate pursuant to RA7160 or the Local Government Code of 1991.
5.2 Puroks. Residents of the puroks, under the leadership of their elected/designated Purok
President/Leader, shall take an active role in mobilizing all forms of support to the implementation
of the program. They are responsible for ensuring the activities/interventions under the program
address their specific needs efficiently and effectively.
SECTION 6. FUNDING SOURCE
The funding for this program is rooted in the principle that public offices are public trust,
embodying the will of the people. This principles lies at the heart of the democracy we uphold today
where both local and national official are accountable to their constituents. The funds to be utilized
in this program are derived from the people and the nation’s natural resources, which must be
preserved for future generation.
6.1. The program shall utilize existing budget allocation from various national, regional,
provincial, and city LGU programs whichever is applicable, that are aligned with the “PuroKalusugan”
Program objectives.
6.2. Additional funding may be sourced from grants and subsidies provided by non-
government organizations and entities.
6.3. Barangay LGU is encouraged to provide funding support to the program;
SECTION 7. MONITORING AND REPORTING REQUIREMENTS
7.1. The Core Lead Offices shall conduct a monthly monitoring of the program’s
implementation. Each office shall submit a monthly report on the progress and accomplishments in
delivering essential government services to the MHO for consolidation and subsequent presentation
to the Municipal Mayor; and
7.2. All partner agencies and entities shall be requested to regularly submit reports on their
activities and accomplishments related to the program.
RESOLVED FINALLY, let the copy furnish of this resolution be on Municipal Local Health Board and
Barangay Development Council, and any governing agencies related to health services.
